Dystonia UK Awards 2024 nomination form

Nominations are now open for the ’Dystonia Superstar’ (England, Scotland, Wales, and Northern Ireland), ’Masked Hero’ and ’Young Champion’ awards.
Nominations will close on 19th May 2024 at 23:59 BST. We will then make a shortlist available for voting, which will include your chance to vote for your most ‘Popular Podcast’ too!
The winners will be celebrated at our Awards ceremony held on Friday 27th September at the legendary Oval cricket grounds in London!

Your 🌟 Dystonia Superstar Award 🌟 nomination

The ‘Dystonia Superstar Award’ is your chance to recognise a superstar in the dystonia community – whether they have dystonia themselves or not, they are someone that has made a difference to you or others. It’s up to you to decide what makes them a Dystonia Superstar. And this year we want to see people from all across the UK! 1 Superstar each from England, Scotland, Wales and Northern Ireland.

Your 🏆 Young Champion Award 🏆 nomination

The ’Young Champion Award’ offers the perfect opportunity to celebrate individuals aged 16-25 who embody extraordinary qualities and deserve some recognition! Whether they are living life with dystonia, demonstrate exceptional support as a sibling or have achieved something you want everybody to know about, now is the moment to acknowledge how outstanding they are. Let’s shine the spotlight on these remarkable individuals and give them the recognition they truly deserve!

Your 🦸 Masked Hero Award 🦸 nomination

The ‘Masked Hero Award’ celebrates a medical professional who has made a difference to your life from doctors, nurses, physiotherapists, and professionals all dedicated to the dystonia community. We want to recognise those special people whose care, attention and commitment have gone beyond the call of duty.

Nomination
1. Submitting the Dystonia UK Awards 2024 nomination form indicates acceptance of our terms and conditions.
2. Individuals cannot nominate themselves in any category.
3. Your chosen nominee must be a resident of the United Kingdom.
4. Your chosen nominee of the Young Champion Award must be aged 16-25 years old at the time of nomination.
5. Dystonia UK reserves the right to move nominations to another category. This is because another category may be more suitable.
6. A valid nomination in a category requires all fields to be completed, providing your name and contact details, the nominee’s name and contact details, and a reason for the nomination. If you submit an incomplete form, we may contact you with the details you provide us to gather any missing information. If we are unable to contact you, your incomplete nomination will not be included in the shortlisting process.

Privacy
1. We will only ever gather and process your personal data (including any special category personal data) provided on the nomination form, in order to ensure that necessary arrangements are made for the Awards process. This may include contacting you to ask you about your nomination, for the purposes of ensuring we have the complete information required for a valid nomination.
2. Where you provide name and contact details of your chosen nominee(s), we assume that you have permission from the individual to share their personal data with us for the purposes of the Awards process. We may contact the nominee(s) directly for the following purposes: to inform them of their nomination, during the shortlisting process, to notify them if they win and to arrange any prizes, to invite them to attend the awards ceremony. This process plans to be concluded by December 2024.
